We are glad that you have chosen to attend Blue Mountain Academy. If you have not done so already, please be sure to familiarize yourself with our school handbook that outlines the principles and policies of BMA.
To apply, please download the forms listed below and fill them out. then fax or e-mail them to Blue Mountain Academy Admissions Office. Once a prospective student’s file is complete, it will be submitted to Admissions Committee for review. In addition to these forms, we may require a face-to-face interview with the applicant. Individual Forms
The following five forms must be on file in the Registrar’s Office before the prospective student will be considered for acceptance to Blue Mountain Academy.
1. Application Form Including Application Fee of $20 and a Photo of Applicant
2. Recommendation Form (3 required)
- 2 Recommendations (current school teacher, principal, etc.)
- 1 Recommendation (church pastor, SS teacher, etc)
3. Transcript Release Form
4. Discipline Form
5. Financial Intent
The following forms are required after acceptance but before a student may attend school. See the Application Packet Checklist for more details.

3. Immunization Form
